Employee premiums are based on your annual salary.  An increase of 10% for single participants and 15% for dependent plans has been added.

The overall cost share of all premiums combined is 80/20, with the largest cost share distributed to higher salary employees.

“Total cost” is the total monthly premium for the benefit, including the employer share.
