We are looking forward to our collaboration with you, and hope that this page helps make it a little easier to navigate the paperwork that is needed when working with a federally-funded research center.
Please fill in the following forms and complete the following steps:
U.S. residents
- Visitor Registration Form - online form required by the Federal Government
- If you are engaging with us remotely, please select "Other" under "Location of Visit." An example response might be "remote talk" or "zoom consultation."
- Direct Deposit form
- Please sign as authorized official at bottom.
- If you'd prefer to receive payment in a physical form, please provide a mailing address.
- Important note: You will likely receive a phone call from Sarah Gibson at NRAO to confirm your ACH deposit information. This is a legitimate call.
- W-9
- Please ensure that the number provided in part I matches the name/entity provided in line 1.
- Please sign under part II.
- Please note that the name on the W-9 and the direct deposit needs to match. I.e. if using your business on the direct deposit form, your W-9 is required to match with the same name.
